Similar to how pain is managed- most of us can deal with OTC but if you experience migraines for instance you might need a little extra something. If you start to see signs of an infection, or want to be preventative after you get a cut or something, a low dose OTC will help you do it sooner. And I understand we are concerned about resistance but at the doses I'm thinking you will need a doctor for anything strong enough to move the needle.
Go off the grid and hope it works, preferably in a location that does not extradite people to whatever country I'm in if possible. Keep a cyanide pill somewhere handy in case it doesn't work.
